Fix row access after materializing query result columns

What

• Allow result_rows after result_columns.

Why

• Reading columns first raises StreamClosedError on later row access, even though the data is cached.

Implementation

• Build rows from cached columns on demand.
• Cover both access orders, empty results, multiple blocks and source cleanup.
• Add a changelog entry.
