PR Description
This implements instructions on including filtering on death.
We've classified as a lack of record of death as alive - though its possible that this means 'unknown/missing record' - we might want to update the instructions to reflect that. CDS will provide guidance that no death record means alive and that the custodian has to flag if the death table is not used (to mean dead/alive status i unknown)
