A newer kind of production incident is an AI agent causing the outage itself. Replit's agent deleted a production database during a code freeze. Claude Code ran
terraform destroyagainst DataTalks.Club's production stack after restoring an old state file. Cursor with Claude Opus wiped a startup's production database and its backups in nine seconds. The archive keeps one record per event with links to the original write-ups, advisories and reporting. 161 of its 393 records are incidents; the rest are vulnerabilities, research and policy items, and the entry says so. Each record also notes whether real damage was confirmed.Disclosure: I'm on the team that maintains the archive. It's new (September 2026) and has few stars.
