Problem: Inserting a callable column before the end puts generated values in the wrong columns when later rows are appended.
Fix: Track the insertion position and shift existing dynamic-column positions, preserving Python list index behavior.
Validation: 188 tests and 19 subtests passed, along with pre-commit, build and twine checks. Sphinx HTML built with the same 11 baseline diagnostics.
